Make sure your passport is really lost or stolen because once you've reported it missing, it immediately becomes invalid and you have to apply for a new one.
If your passport is lost or stolen, report it immediately to help prevent identity theft. Submit Form DS-64 online or by mail.
You can report your passport lost or stolen three ways:
- Login into the US State Department website and navigate to the Report Lost or Stolen tab and submit Form DS-64.
- Fill out and mail in Form DS-64.
- Call 1-877-487-2778 (TTY 1-888-874-7793) and report.
You are starting over as if you were applying for your first passport.
You must apply in person for your new passport. Where you are and how soon you need your passport determines where you should apply. See the list below.
Don't forget to bring a filled out Form DS-64 with your application if you called to report your passport lost or stolen.
